What Are Ultrasound-Guided Injections?

An ultrasound-guided injection is an injection into a joint or soft tissue that is performed while the clinician watches the area on a live ultrasound screen, so the needle is placed under direct vision rather than by surface landmarks alone.

Ultrasound is a scanning method that shows soft tissue — tendons, ligaments, joint spaces and the fluid around them — in real time and without radiation. Used during an injection, it lets the clinician see the target structure and the needle at the same time.

The technique is a way of delivering an injection, not a treatment in itself. What is injected — and whether an injection is appropriate at all — depends entirely on what the assessment finds.

Who Are Ultrasound-Guided Injections For?

Guidance is generally considered where the target is small, deep or close to other structures — for example within a joint space or around a tendon sheath — and where the clinician wants to confirm placement directly.

Suitability is assessed case by case. Whether an injection is the right step, and whether guidance is needed for it, is a clinical judgement made after examination and any imaging your specialist asks for. For some presentations an injection is not the appropriate route, and we will say so.

Who it’s not for

A few situations mean it’s better to wait, or to choose a different treatment:

  • Active infection in or around the area to be injected
  • Bleeding disorders, or blood-thinning medication, depending on the medicine and the site — disclose everything you take at consultation
  • Known allergy to any component of what is being injected
  • Pregnancy or breastfeeding, depending on what is proposed

    A specialist takes your history, examines the area and reviews any imaging you have, then sets out whether an injection is an appropriate option for you and what the alternatives are.

    The scan

    Ultrasound gel is applied and the clinician scans the area to identify the target structure before anything is prepared.

    The skin is cleaned and the injection is performed while the clinician watches the screen. Most people describe pressure rather than sharp pain; local anaesthetic may be used depending on the site.

Ultrasound shows the clinician the target structure and the needle while the injection is performed, so placement is confirmed directly rather than estimated from surface landmarks. What that means for any individual is a question for your specialist — we do not make outcome claims for injections on this page.

That depends on the assessment and is decided with you at consultation. Guidance is the delivery method; it is separate from the decision about what, if anything, is appropriate to inject.
