What it is
Plasmapheresis is a therapeutic procedure that separates whole blood, removes a portion of the plasma and replaces it with a substitute fluid before returning the blood to the body. It is also called therapeutic plasma exchange.
The plasma, which is the fluid portion of the blood, is drawn off using a specialised machine while the red cells, white cells and platelets are retained. A portion of the plasma is then removed and replaced with a substitute fluid before the blood is returned to the body.
At Shookra the procedure is carried out under physician supervision, with the volume of plasma exchanged and the replacement fluid determined by your clinician. The session is conducted in a clinical setting with monitoring throughout, and the protocol is set after a review of your history and the goals discussed at consultation.
The intended rationale is to reduce the circulating concentration of certain substances carried in the plasma; whether it is appropriate for a given individual is a clinical decision made by the supervising physician.

Who it's for
Plasmapheresis is a clinical procedure rather than a general wellness treatment — usually considered by adults exploring physician-led longevity protocols, people referred after a clinical assessment, or those weighing it carefully alongside other longevity options.
Suitability is never assumed. Plasmapheresis is not appropriate for everyone, and your physician confirms whether it is right for you at consultation — including a review of your medical history, current medication and any contraindications.
- Inflammatory or autoimmune load
- Persistent fatigue
- Brain fog and difficulty focusing
- Slow recovery
- Immune support
Who it’s not for
A few situations mean it’s better to wait, or to choose a different treatment:
- Pregnancy or breastfeeding
- A bleeding or clotting disorder, or use of anticoagulant medication
- An active infection
- Cardiovascular instability, or a known allergy to the replacement fluids
This isn’t exhaustive, and none of it rules you out on its own — your clinician confirms suitability at consultation.
What to expect
- 01
Assessment & consent
Your physician reviews your history and goals, explains the procedure and the replacement fluid used, and confirms suitability before any treatment is planned.
- 02
The exchange
Blood is drawn and passed through a separator that removes a portion of the plasma; the remaining components are returned with a replacement fluid, with monitoring throughout the session.
- 03
Observation & aftercare
You are observed before leaving, and your clinician gives aftercare guidance and discusses whether any follow-up is indicated.
Physician note
On Plasmapheresis
Plasmapheresis is a procedure I approach conservatively. I want a clear clinical rationale before recommending it, and I prefer to set expectations plainly: it is performed under supervision, suitability is assessed individually, and it sits within a wider longevity plan rather than standing alone. If it is not the right option, I will say so.
